The Meadows opened in December 1998, as a full-service, 97-bed, long term care facility located just south of I-275 at the Winton Road Exit in Forest Park. The facility is dedicated to providing quality care for our residents by creating a supportive, respectful and home-like environment that encourages the highest level of functioning for each individual resident. Care is delivered by licensed and certified staff in their respective disciplines.
